Labor Standards Division Announces Updated Forms

The Missouri Labor Department’s Division of Labor Standards (DLS) has recently updated some of its forms that are used regularly by contractors and sub-contractors. The revised forms are provided below, and the DLS encourages you to access the forms and check out the new requirements.

• Project Notification - Contractor Information Notification (PW-2)  (size 72k)
• Affidavit of Compliance with Prevailing Wage Law  (PW-4)  (size 57k)
• Missouri Department of Labor Contractors Payroll Record (LS-57)
